Informed Consent Notice

My name is David Keathly and I am a Lecturer in the Computer Science and Engineering Department at the University of North Texas, and Co-Director of the Robocamp and EChallenge programs in which your daughter participated. I am conducting a study about our camp as part of a research and reporting effort for the Technology Workforce Development program and the many foundations that supports our camp, and as part of a conference paper being prepared for the American Society for Engineering Education Annual Conference 2008. These foundations and grant programs are crucial to maintaining the funding levels for the camp that bring it with no charge to your daughter (and sons in 2008!)

If you agree to take part in this study, you will be asked to complete a questionnaire about your daughter's experiences at Robocamp or EChallenge camp and plans for future study. There is also a survey to be completed by your daughter. It will take approximately 15 minutes to complete. Participation in this study may not benefit you personally. However, your responses may help us learn more about how to make our programs better and to secure continued funding for Robocamp and similar programs for women and minorities. I would appreciate your responses by April 15, 2008 in order to meet the deadlines for paper submission.

Participation in this study is completely voluntary. You have the right to skip any question you choose not to answer. Your name will not be requested in this study so your responses will be anonymous. All research records will be kept confidential, including email addresses and other personal information. No individual responses will be disclosed to anyone because all data will be reported on a group basis. If you have any questions about the study, please contact me at 940 565-4801 in the Computer Science and Engineering Department.

This research project has been reviewed and approved by the UNT Institutional Review Board. Please contact the UNT IRB at 940-565-3940 with any questions regarding your rights as a research subject.

2. Which camp programs has your daughter attended and during which years?


3. Is your daughter planning to attend college? If so has she chosen a school and a major? What are they?


4. Does your daughter have a strong interest in math, science or engineering topics?


5. Has her overall performance and interest in math and science improved as a result of her experiences at Robocamp?


6. Would your daughter be interested in an advanced level Robocamp if one were offered?


7. If a scholarship were available for Robocamp graduates to attend UNT in math, science or engineering would your daughter be interested?


8. If your daughter is not interested in math, science or engineering as a course of study and career, what area of study and work does she plan to pursue?


9. Do you feel that the Robocamp experience had any other positive or negative impacts on your daughter's performance at school or otherwise?If so please explain.
